Once you have made contact with us, one of our trained Managers will arrange a convenient time to visit you to discuss your needs and requirements. Our aim is to involve you the individual and your family right from the beginning, including the recruitment process, assessment and care planning.
The Manager will listen carefully and include you whilst he/she completes a registration form with all of your personal details. Whilst at your home the Manager will carry out a risk assessment to make sure any concerns are addressed before the service is offered.
On return to the office the Manager will then produce a Care Plan to suit your individual needs and will then return to discuss this with you so as to ensure you are comfortable with the package being offered.
Once the Service is in place then the whole package will be monitored through phone calls, home visits and staff supervision. Should you at any time decide you would like to change any aspect of your Service then this can be discussed and actioned appropriately. It is important to us that the Care Plan we suggest can really work to allow you to attain your personal goals with the support from our team of professionals we believe this is both measurable and achievable.
